- What is MyChart?
MyChart is an online portal that offers personalized and secure access to portions of your medical records. It enables you to use the Internet to manage and receive information about your health. With MyChart, you can:
- Request medical appointments
- View your health summary from the MyChart electronic health record
- View test results
- Request prescription renewals
- Access trusted health information resources
- Communicate electronically and securely with your medical care team
- Is there a fee to use MyChart?
No. MyChart is a service that is offered to our patients free of charge.
- How do I sign up for MyChart?
You can obtain a MyChart access code from your participating MyChart doctor’s office, on your hospital discharge paperwork, or from the Memorial Healthcare System Health Information Management (HIM) Department. You will use the access code to create a user ID and password on the website.
- Whom do I contact if I have further questions?
Your participating MyChart doctor’s office or the Memorial Healthcare System Health Information Management (HIM) Department is available to answer your questions. If it is outside of normal office hours, or if you need further assistance, you may contact the Memorial Healthcare System Help Desk at 954-276-7676.

- When can I see my test results in MyChart?
Your test results usually are available in MyChart within three (3) business days. Certain restrictions apply.
- What kind of test results can I view in MyChart?
You will be able to receive most laboratory and radiology results. Test results for sexually transmitted disease and alcohol and substance abuse will not be available in MyChart.
- Will my test results in MyChart include any notes or explanations from my participating MyChart doctor’s office?
Your participating MyChart doctor may choose to write a note to accompany your test results in MyChart.
- What should I do if some of my health information on MyChart is not correct?
Your MyChart information comes directly from your electronic medical record. To correct any inaccurate information regarding your hospital records, contact your doctor’s (MyChart Provider’s) office regarding your doctor’s records or Memorial Healthcare System Health Information Management (HIM) Department. Your health information is reviewed and updated in your electronic medical record during each visit.
- What types of messages can I send to my participating MyChart doctor’s office through MyChart?
MyChart Messaging offers to you the convenience of requesting appointments, renewing prescriptions, and communicating non-urgent matters.
- If I send a message to my doctor, when can I expect a reply?
Generally, answers are provided within five business days. Please note that MyChart should not be used for urgent situations. Please contact your medical center if the situation requires immediate attention or dial 911 if it is an emergency.

- Can I view a family member’s health record in MyChart?
Yes. This is called proxy access. Proxy access allows a parent, guardian or designee to login to his or her own personal MyChart account and connect to information regarding the family member. Proxy access can be used to view health records for children and adult patients. Complete the appropriate Proxy/Authorization form(s) and return it to your participating MyChart doctor's office or Memorial Healthcare System's Health Information Management (HIM) Department within the hospital to request access to this convenient service. You can obtain the proxy consent forms in the upper left corner of the screen under Proxy Consent Forms.
The rules for accessing a child’s account change on the child’s 13th birthday. This change occurs because state and federal law protect the privacy of pediatric patients seeking medical treatment for pregnancy, sexually transmitted disease, substance abuse, and mental health issues. Memorial MyChart is available to the teenager’s themselves age 13 through 17. In order for a parent or legal guardian to access a teenager’s MyChart health information, the teenager must consent to the disclosure. Teen MyChart Access is available only to teens who grant consent to a parent or legal guardian. A teen may revoke this consent at any time. Teen MyChart Access expires at age 18. On your teen’s 18th birthday, the Parent Proxy view will be terminated automatically.
- Can I ask questions regarding a family member from my MyChart account?
The proper way to talk to a provider about a family member is by using the proxy access feature of MyChart.
- Can my spouse and I share one MyChart account?
No. Due to the sensitive nature of medical information, each adult must sign and submit proxy consent forms and establish his or her own MyChart account. Please click on the link in the upper left corner of the Memorial MyChart home page to download the appropriate proxy form.

- I forgot my password. What should I do?
You may contact your physician’s (MyChart Provider’s) office to request a new, secure password, or click here to reset your password online.
- I lost my access code, let it expire, or did not receive it. Can you send a new access code to me?
To obtain a new access code, you must request one in person from your participating MyChart Provider’s office or the Memorial Healthcare System Health Information Management (HIM) Department.
- Where can I update my personal information (e.g., e-mail or change my password)?
Login to MyChart, and from the left menu, go to the Preferences section and select the appropriate option.

- How secure is MyChart?
The Memorial MyChart website uses encryption software. While we use state-of-the-art security, no system can guard perfectly against risks of intentional intrusion or inadvertent disclosure of MyChart. When using Memorial MyChart, information will be transmitted over a medium that is beyond the control of Memorial Healthcare System and its contractors. YOU HEREBY EXPRESSLY ASSUME THE SOLE RISK OF ANY UNAUTHORIZED DISCLOSURE OR INTENTIONAL INTRUSTION, OR OF ANY DELAY, FAILURE, INTERRUPTION OR CORRUPTION OF DATA OR OTHER INFORMATION TRANSMITTED IN CONNECTION WITH THE USE OF THIS SERVICE. Once we receive your medical information, it will be treated as confidential, with the same protection as all other Memorial Healthcare System medical records.
- What is your privacy policy?
- MyChart is owned and operated by Epic and is fully compliant with federal and state laws pertaining to your privacy. Your name and e-mail address will be treated with the same care and privacy that is given to your health records, and will never be sold or leased by MyChart.
- I was logged out of MyChart. What happened?
- If your keyboard remains idle for 15 minutes or more while you logged into MyChart, you will be logged out of MyChart automatically. We recommend that you log out of your MyChart session if you need to leave your computer for even a short period of time.
- What do I need in order to use MyChart?
- You need access to a computer, an Internet connection, and an up-to-date browser.
- My access code does not work. What should I do?
For your security, your access code expires after 60 days and is no longer valid after the first time that you use it. If your code has expired, contact your physician’s (MyChart Provider’s) office or Memorial Healthcare System Health Information Management (HIM) Department to obtain a new access code. If you are having problems logging in, contact the Memorial Healthcare System Help Desk at 954-276-7676.
- Are my access code and user ID the same?
No. Your access code is different than your MyChart ID and password. Your access code will be used only to log into MyChart for the first time. When you log into MyChart the first time, you will be asked to create your own unique MyChart ID and password. Your access code will expire within 60 days, if not used.
- Can I access MyChart through a mobile device?
Yes. MyChart can be accessed through an iPhone or Android device. The mobile application has most, but not all, of the MyChart functionality. You must activate MyChart on the website prior to using the mobile application.
- I am having trouble while trying to log in. What should I do?
If you have forgotten your user ID, click here to recover it.
If you have forgotten your password, click here to reset it.
